Requirements for New Students

1. Photocopy of Birth Certificate
2. Medical Certificate (indicates that the child is fit to go to school)
3. Eye Doctor’s Diagnosis (at least 3 months from date of enrollment)
4. Dental Check-up (at least 3 months from date of enrollment)
5. 3 copies of 1 x 1 photo of the child
6. 2 copies of 1 x 1 photo of Escorts (each authorized Escort must submit photos)
7. Family Picture (The photo is displayed in the Preppi Family Corner)
8. Sample skirt/shorts for uniform size

Additional Requirements for Special Needs Students

Present the following:
1) Brief Bio of your child (name, age, sex, educational background, etc.)
2) When was your child assessed with special needs?
3) What has been done since the Assessments?
4) Present pertinent documents for review: past and recent
a) Doctor’s Diagnosis and Medications prescribed
b) Therapists’ Reports
c) Teachers’ (School’s) Reports
5) Provide your personal narrative about your child (your observations, realizations, methods you discovered that works best for your child, phobias, sources anxieties, favorites, causes of tantrums or uncontrolled behavior, etc.)
6) Discuss your Aims for your child (What you expect and hope for from the Preppi Education (i.e. learn to obey, control emotions, acquire reading skills, etc.)

Requirements for Returning Students

1. Medical Certificate (indicates that the child is fit to go to school)
2. 2 copies of 1 x 1 photo of the child
3. 2 copies of 1 x 1 photo of Escorts (If there are changes in the Escort Profile)
4. Sample skirt/shorts (if a new uniform needs to be made)
5. Updated Medical Records (i.e. Doctor’s Assessments) if the child needs Special Attention and has Special Needs

Additional Requirements For International Students and Transferees (in itals only)

1. Copy of student’s Passport (note validity of date)
2. Copy of Parents’ or Guardian’s Passport with valid Work Permit in the Philippines
3. Copy of Visa or document of valid stay (note the duration of stay indicated on the passport)
4. SSP form
5. Affidavit of Support from parents or guardian
6. Affidavit of Parents appointing the guardian
7. Transcript of Record of 2 previous years (It should have a note on the promotion of the child to the next level)
8. Letter of Recommendation from previous school with Certification on Good Moral Conduct
9. Medical Records (i.e. Doctor’s Assessments) if the child needs Special Attention and has Special Needs
